What goes in each section?
The Cornell Learning Strategies Center's outline describes recording notes, adding questions, covering the notes to answer those questions, reflecting and reviewing. Our printable is an original layout for that workflow.

Leave room between ideas. If a lecturer moves from one concept to another, start a new block or page. The template works better when a question sits beside the part of the notes that answers it.
A completed Cornell notes example
Page 2 uses a short, illustrative mathematics lesson: reading the linear function y = 2x + 3. It is deliberately small enough to show how the columns connect.
- Question: What does the slope tell me? Notes: The slope is 2. Increasing x by 1 increases y by 2.
- Question: What is the vertical intercept? Notes: At x = 0, y = 3, so the graph crosses the y-axis at (0, 3).
- Question: What is y when x = 4? Notes: Substitute 4: y = 2(4) + 3 = 11.
The summary connects these pieces: the slope describes the rate of change, the intercept gives the starting height at x = 0, and substitution checks points on the graph. A summary that only lists “slope, intercept, substitution” would give you less to work with next week.
To test whether you can apply the idea, change the example to y = 3x + 1. Identify the slope and intercept, then find y when x = 4. The answers are 3, 1 and 13. This extra problem checks whether you can use the pattern rather than repeat the completed page.
Use the PDF on paper or a tablet
For a printed notebook
The file uses A4 pages. Select page 1 in your printer dialog and print as many blank copies as you need. If your printer uses US Letter, choose the option that fits the page to the paper; the writing areas will be slightly smaller. Check the preview before printing a large batch.
Keep each module's pages together and number them. When an example needs more space, continue on a second sheet with a matching topic label. A separate blank page is often easier for a long derivation than squeezing every step into the notes column.
For digital handwriting
Save the PDF and open it in an app that supports PDF annotation. Keep a clean copy, then duplicate or reimport it for each lecture using that app's controls. The download is a static writing template, not a form with clickable fields. A stylus or the annotation app's text tool supplies your notes.
You can also reproduce the structure in a document editor: a two-column table with a wide notes column, followed by a summary box. Exact proportions matter less than having enough room to write and being able to hide the answers.
Turn the completed page into a review task
After class, choose three questions from the page and cover the notes column. Answer aloud or on spare paper, then check. Mark each answer as complete, incomplete or uncertain. Keep a separate mark for an idea you never understood so you can ask for an explanation.

Do Cornell notes work for every lecture?
Use the template where questions and explanations fit naturally. Large diagrams, proofs or studio notes may need an open page. You can still add review questions beside that work.
